Senior Consultant, Population Health & Care Transformation

What you'll do

• Facilitate customer working sessions to identify goals, workflows, reporting needs, and implementation requirements.
• Translate requirements into documented workflow recommendations, configuration needs, and delivery activities.
• Own assigned implementation workstreams by maintaining clear deliverables, decisions, and status updates.
• Perform functional and data validation and support user acceptance testing to investigate discrepancies.
• Apply population health and quality-reporting knowledge to deliver technical solutions and requirements.
• Identify risks and blockers early and escalate issues with supporting evidence and customer impact.
• Develop and deliver customer training to assess readiness and support go-live adoption.
• Coordinate customer feedback with product teams to improve reusable documentation and delivery practices.

What we're looking for

• Must have 3 to 5+ years of experience.
• Must be able to read, write, and speak English.
• Must possess consulting and software implementation skills.
• Must have experience in requirements gathering and documentation.
• Must have experience in solution implementation services and validation.
• Must possess skills in planning, organizing, and issue escalation management.
• Must have knowledge of population management and process improvement.
• Must possess impactful communication skills.
