What you'll do

  • Conduct comprehensive assessments to develop time-sensitive treatment plans for DoW beneficiaries.
  • Coordinate healthcare resources and facilitate referrals to appropriate health care providers or facilities.
  • Oversee discharge planning activities to ensure optimal client care transitions.
  • Track progress, evaluate outcomes, and adjust individualized care plans as needed.
  • Serve as the central point and lead for tracking service members into cross-functional teams for multi-domain services.
  • Collect, analyze, and report data on multi-domain initiatives to higher-level authorities.
  • Provide professional assistance with health care resources to identify clients' needs and enhance their level of function.

What we're looking for

  • Minimum 2-year degree as a registered nurse or Master's in social work from an accredited school.
  • At least two years of nursing experience or case management.
  • Certified as a case manager within one year of hiring.
  • Knowledge of VA/DoD Clinical Practice Guidelines and healthcare financial environment.
  • Understanding of pharmaceuticals, community resources, and third-party regulations.
  • Professional communication skills for briefings, presentations, and research.
